Registration with the Practice

PreRegistration

Ask to register for appointments online

You will be given a unique username and password - along with details of how to gain access to the website and the NHS App.

To register for this you need to complete the online registration form and our new patient health questionnaire. We do require proof of ID to register you for this service.

If you experience any problems using this service contact the surgery for assistance.

Documents requested for registration

  • passport (if applicable)
  • asylum seeker card (if applicable)
  • proof of name & address e.g. Utility bill (less than 3 months old), council tax bill, driving licence, bank statement
  • completed health questionnaire
  • children under 16 cannot register without guardian/family
  • red book (for all children under 6)

If your circumstances mean that you are unable to provide the above documentation. Please let the surgery know when you provide your completed registration forms.

When you register you will also be asked to complete a medical questionnaire, the Queenhill New patient Questionnaire below.   This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may complete and send to us via email or as a physical copy:

Note that by sending the form you will be transmitting information about your self across the Internet and although every effort is made to keep this information secure, no guarantee can be offered in this respect.


Patients who are 16 and over and under 18 years of age

You have now reached the age where you are entitled to confidentiality.

You may wish to have your own contact details on your records. This will mean that when the surgery contacts you we will be speaking directly to you rather than involving someone else in your care.

If you wish to keep the same family details this is fine and the surgery will always respect your confidentiality.


Registering with us without using the online service?

Alternatively you may print off a NHS registration form, along with the Queenhill New Patient Questionnaire above, complete both forms and bring with you on your first visit to the practice.
